PURPOSE:
The Population Health Strategy Lead Consultant serves as the primary liaison between CareFirst and several key health care delivery partners, assigned several health systems with single hospitals, generally covering specific geographies surrounding the anchor hospital. The Population Health Strategy Lead Consultant is responsible for driving transformation of the health care experience for CareFirst Members through the health care delivery partner(s) they are assigned to manage. The goal of the incumbent is to improve value and outcomes. This role ensures that the Consultant teams are conducting actionable analysis and driving behavior change within entities including identification of care opportunities, providing patient outreach, participating in performance/ process improvement efforts, and recommending areas of opportunity to support CareFirst Value Based Programs.

ESSENTIAL FUNCTIONS:
  • In collaboration with executive-level leaders at assigned health systems, develops and executes the agenda and business plan. Works with internal CareFirst resources and the health care delivery partners resources to execute the plan. Leverages strong strategic relationship with the entity leadership to quickly resolve any problems so that the agreed upon objectives are met.
  • Leverages their level of involvement and entity relationships to identify opportunities to align initiatives already in progress with things that would benefit the entity in Value Based Programs, thereby accelerating the Value Based Programs promote and the results we seek.
  • Serves as CareFirst's internal expert on the assigned health care delivery partners including knowledge of the partners strategic plan, financial health, geographic footprint, mix of service lines offered, executive leadership and board make-up. Maintains day to day contact with the entity's leadership and assures that problems are addressed, and practice transformation plans stay on course.
  • Provides the entity with information on enhancements and changes to CareFirst Value Based Programs. Troubleshoots or explains data that was questioned and identifies opportunities for alignment between the entity and CareFirst.
  • Analyzes and reports on progress amongst the entity, preparing impact analysis and presentations for senior executives. Provides briefings on how the entity is doing as a whole, which areas are contributing or detracting from the entity's success in the program, the status of key initiatives. Regularly briefs VPs, EVPs and CEOs from CareFirst and the health care delivery partner on progress, barriers and opportunities.
SUPERVISORY RESPONSIBILITY:
Leads a team utilizing a matrix management system.

QUALIFICATIONS:

Education Level: Master's Degree in Public Health, Health Administration, Business Administration, or related field or equivalent OR in lieu of a Master's degree, an additional 6 years of relevant work experience is required in addition to the required work experience.

Experience:
  • 5 years progressively increasing responsibility in health care environment, health insurance payer environment, or related industry.
  • 3 years Field experience in the role as Population Health Strategy Consultant within CareFirst.
Preferred Qualifications:
  • Consulting skills and/or direct experience in a consulting role.
  • Experience in a clinical setting or knowledge in a clinically related field.
  • Project management experience.
  • Experience using Client Relationship Management (CRM) System(s).

  • Must be able to meet established deadlines and handle multiple customer service demands from internal and external customers, within set expectations for service excellence. Must be able to effectively communicate and provide positive customer service to every internal and external customer, including customers who may be demanding or otherwise challenging.
Salary Range: $86,328 - $171,457

Travel Requirements
Estimate Amount: 80% Able to travel through assigned region on a daily basis and work in the field the majority of workdays.
