Position Summary: We have an exciting opportunity to join our team as a Field Nurse, Garden City/Nassau - Home Care. In this role, the successful candidate Provides safe, quality, therapeutic age appropriate nursing care using evidence based practice. Uses clinical expertise in assessment, clinical decision making, clinical practice and patient education. The nurse coordinates the care provided with the interdisciplinary team to promote optimum patient outcomes and satisfaction. Job Responsibilities:
Additional Position Specific Responsibilities: Palliative Nurse Certification or working toward Certification Minimum Qualifications: To qualify you must have a Bachelor's degree in Nursing required for employees hired after 9/1/2020 (those who were hired prior to 9/1/2020 are grandfathered). Current and valid NYS Registered Professional Nurse license and registration. 1- 2 years Acute care facility OR Home Health,Hospice, and Long Term Home Health Care Program. Excellent communication (both oral and written), interpersonal, customer service skills. Critical thinking skills. Working knowledge of Computers. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
