Summary:

The Program Coordinator is responsible for the development, coordination and implementation of the Fostering Health Program protocol and intake process. The work promotes community relationships through collaborative partnerships working with diverse populations, prioritizing high-risk children and families. The Program Coordinator serves as a resource to patients, families, the community leadership and system partners to identify target audiences to communicate program goals and accessibility. Requires some weekend/evening work and occasional travel in the ten-county Region 3B service area as assigned by The Department of (Tarrant, Johnson, Hood, Parker, Wise, Denton, Palo Pinto, Erath, Cooke).

Education:
  • Requires a Master of Social Work degree.
  • Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions.
  • Bilingual English and Spanish a plus.
  • Required skills: relationship-building, effective written and verbal communication, critical thinking and problem-solving, ability to plan and organize effectively, proficient in use of computer applications.

Certification/Licensure:
  • Licensed Master Social Worker current, unrestricted Texas license.
