Position Summary:
We have an exciting opportunity to join our team as a Project Coordinator - Brooklyn.

In this role, the successful candidate Position Summary: Coordinates directs and organizes all aspects of primary and behavioral care provided the public health RN and patient navigators at designated homeless shelter sites as well as oversee daily operations of Bedford Atlantic Shelter Serve as liaison between clinic health team DHS Office of the Medical Director shelter staff director Community Medicine Program NYU FHC and other agencies Job Responsibilities: Participate in CMP management meetings weekly by phone or in person Participate in weekly meetings with the shelter staff and Department of Homeless Services Office of the Medical Director Oversee the assignments and workflows of the RN and patient navigators assigned to the Nurse in shelter grant funded project Develop and oversee shelter outreach program working with shelter leadership case management staff and intake staff to coordinate medical and mental health services as extensively as possible in all shelter intake processes and monitor performance of same Monitor and ensure appropriate documentation of all contract deliverables to ensure compliance Generate statistical reports relating to Nurse in shelter grant funded project and as requested by Department of Homeless Services/Office of the Medical Director and with the approval of CMP senior management Generate statistical reports relating to Nurse in shelter grant as requested by other members of CMP management and NYU FHC management for grant proposals decision support and quality management and improvement Liaise with IT/HIM to identify and correct any hardware and software issues Coordinate program wide transition to NYU FHC EMR and IT environments Responsible for implementation of quality improvement staff satisfaction and patient satisfaction initiatives for nurse in shelter sites Liaison to Community Medicine Management for any concerns issues or opportunities at the various sites for the nurse in shelter grant Administrative oversight of all staff onsite including clerical clinical support staff counseling staff and providers Additional Position Responsibilities: serve as back up for the patient navigator staff to ensure proper staffing at all sites for the nurse in shelter grant Minimum Qualifications: Masters Degree Public Health or related field or equivalent combination of education and experience Three years healthcare administration experience; comfortable working in a homeless shelter environment and comfortable making contact with homeless individuals in general Preferred Qualifications: Good communicator highly organized preferably with direct patient/community outreach experience Some supervisory experience desired Experience with EPIC EMR Excel statistical analysis software preferred Working Conditions: Flexibility with work location and willing to travel between locations to various shelter sites Ability to access all areas of the institution Ability to see or access information in print and or transmitted electronically Ability to hear oral instructions Performing job duties may require prolonged periods of sitting standing walking Spends majority of time in the field in either clinic shelter or community settings

Minimum Qualifications:
To qualify you must have a Bachelors Degree Public Health or related field or equivalent combination of education and experience Three years healthcare administration experience; comfortable working in a homeless shelter environment and comfortable making contact with homeless individuals in general Masters Degree highly preferred

Preferred Qualifications:
Good communicator highly organized preferably with direct patient/community outreach experience Some supervisory experience desired Experience with EPIC EMR Excel statistical analysis software preferred

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $62,400.00 - $65,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
