Position Summary:
We have an exciting opportunity to join our team as a Patient Navigator.

In this role, the successful candidate Patient Navigators works with multi disciplinary team in delivering primary care services to the homeless people at designated outreach sites and clinics To accompany clients to various appointments creating appointments both within and outside of NYU Langone Brooklyn and maintain linkage of care between patients

Job Responsibilities:
  • Collect monitor and submit statistics on a weekly basis to ensure we meet funders deliverable and monitor HRSA indicators
  • Arrange for ACA Medicaid expansion education forums for patients program staff shelter staff to ensure patients are duly informed and enrolled
  • Maintain a positive working relationship with HCH team members ER staff and community partners
  • Educate hospital and community partners about unique needs and issues related to homelessness and access to health care
  • Assist patients to sustain efforts toward recovery harm reduction and relapse prevention promoting access to addiction care self help groups and treatment programs in the community
  • Maintain patient linkages with care providers and partner agencies promoting adherence to medical regimens plans of care and achievement of self management goals
  • Facilitate patient navigation through systems of health care and social services providing for public transportation and escort to appointments as needed and as available
  • Assist in the planning and implementation of strategies to facilitate referrals transitions and communication among and between systems of care on behalf of patients who are homeless
  • Conduct outreach in reach case finding and education services to identify and serve individuals who are in need of healthcare and other services
  • Follow up with patients and providers to ensure appointments were kept
  • Assist with patient registration
  • Creating appointments within or outside of the NYUFHC network for CMP patients nbsp;
  • Administrative duties including registering patient charts in EPIC and facilitating telehealth visits
  • Ensures patients make and keep appropriate health and social services appointments; accompanies patients to primary care appointments and to other health care and social service encounters as appropriate
  • Provides education risk reduction counseling and skill building services to the patients as necessary per patient needs and grant requirements
  • Depending on grant requirements and population served may provide rapid HIV testing and pre and post test counseling
  • Performs entitlement reassessment and follow ups
  • Coordinates ongoing navigation and logistical support for appointment keeping reminders transportation and childcare arrangements as appropriate
  • Provides critical information to other members of the health care team based on observations in the field or in patient encounters
  • Educates coaches and empowers patients
  • Depending on needs may provide DOT Daily Observed Therapy ; if providing DOT must observe and record patient self administration of medications
  • Participates in trainings conferences meetings and quality improvement efforts as assigned

Minimum Qualifications:
To qualify you must have a High school diploma Bachelors degree preferred or equivalent combination of education and patient navigation/case management experience Three months healthcare related experience outreach or navigation; comfortable working in a homeless shelter situation and comfortable making contact with homeless individuals in general

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $47,308.39 - $50,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
