What You�ll Do:

• Partner with pediatricians, nurses, and therapists to support comprehensive patient care

• Empower families and caregivers by connecting them with vital services and support networks

• Address social determinants of health and remove barriers to care for children in need

REQUIRED QUALIFICATIONS

Education: Master�s Degree in Social Work

Skills:

� Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills

� Ability to work in self-directed environment with attention to detail, follow through, and has a high level of computer literacy.

� History of positive rapport with patients, families, physicians, and interdisciplinary team

License: Active LISW license

PREFERRED QUALIFICATIONS

Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty is preferred.
