Position Summary: We have an exciting opportunity to join our team as a Home Care Intake Nurse - Per Diem. In this role, the successful candidate completes a comprehensive assessment of patients referred to the Agency for Home care services from Hospitals, skilled nursing facilities, rehab facilities and from the community. Responsible for the planning to admit the patient to the Agency. Determines and ensures client eligibility and authorization to receive requested services and notifies all pertinent staff of approval or denial of requests. Job Responsibilities:
Minimum Qualifications: To qualify you must have a Diploma OR Associates Degree Nursing. Current and Valid New York State Registered Professional Nurse license. 1 - 2 years Acute Care Facility or Home Health, Hospice and/or Long Term Home Health Program. Excellent communication (both oral and written), interpersonal, customer service skills. Critical thinking skills. Working knowledge of computers. Preferred Qualifications: Bachelor's Degree Nursing. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
