Resp & Qualifications PURPOSE: The Clinical Pharmacist Associate will serve as a medication expert supporting a broad range of member facing professional activities and responsibilities. This position is responsible for the comprehensive management of member medication reviews, patient education, and medication care plan development and tracking. The Clinical Pharmacist Associate will collaborate with nurses, social workers, and physicians to integrate services for the best possible care of CareFirst FEP members. The Clinical Pharmacist Associate's services are comprised of individual interventions each of which is intended to elicit a change in patient's drug therapy, reduce the incidence of adverse drug events, and improve adherence to medication regimens. ESSENTIAL FUNCTIONS: Performs data collection to identify referrals for clinical pharmacy programs as well as telephonic therapeutic consultations to patients and/or prescribers. Completes a pharmacotherapy assessment of patient's claims, past medical history and disease states. Provides clinical recommendations pertaining to, but not limited to, adverse events, therapeutic duplication, adherence to therapy, polypharmacy, high risk medications, therapeutic substitution, and untreated disease or conditions. Utilizes formulary information and member specific benefits to guide appropriate medication recommendations. Documents plan of care into relevant databases. Develops a plan of care that details actions of the patient and/or prescriber based on the Pharmacotherapy assessment. Supports quality improvement efforts for pharmacy-related HEDIS measures. Documents interventions into relevant databases and tracks acceptance rates over time. Works with the quality team to improve data collection and reporting in support of quality areas of focus. Tracks productivity, quality, impact, and overall performance of pharmacist interventions. Identifies areas of opportunity for pharmacist interventions. Core elements of care plan documentation may include patient demographics, subjective information, assessment, medication record, and medication recommendations. Develops and reports on pharmacist activities including but not limited to number of referrals, reasons for referrals, number and types of medication recommendations, success of outreaches and implementation of medication changes. Collaborates with care teams to identify appropriate member referrals for the pharmacist and discuss care plans. Helps develop the process of care for the pharmacist including protocols, policies, and procedures. Recommends process improvements, cross functional care coordination best practices and more in support of improving member outcomes. Develops and provides formal written and verbal presentations or other communication materials to support the Clinical Pharmacist Associate's activities in collaboration with cross functional internal care coordination teams. Participates in innovative solutions and clinical programs designed to meet pharmacy needs. QUALIFICATIONS: Education Level: PharmD, Pharmacy (Doctorate of Pharmacy) Experience: 3 years of experience in clinical pharmacy, preferably in a managed care environment Licenses/Certifications PHARMD - Licensed Pharmacist Active in any state and/or DC Registered Pharmacist Upon Hire Required Candidates must live in the MD/DC/VA area in order to travel into the offices a few times per month (or more frequently as needed). Preferred Qualifications: Experience in Managed Care. Experience working in healthcare industry with a thorough understanding of prescription benefit management programs and practices. Formal post-graduate residency training and/or board certification strongly preferred. Knowledge, Skills and Abilities (KSAs) Ability to work independently, as well as a member of a team. Strong critical reasoning skills in clinical decision-making, planning and organizing. Current knowledge of clinical practices and related information systems and computer applications, required. A strong balance between clinical expertise and business acumen. Strong interpersonal, written and verbal communication skills, including presentation skills. Must be able to meet established deadlines and handle multiple customer service demands from internal and external customers, within set expectations for service excellence. Must be able to effectively communicate and provide positive customer service to every internal and external customer, including customers who may be demanding or otherwise challenging. Salary Range: $94,248 - $187,187 (salaries are based on education, years of experience & internal equity) Salary Range Disclaimer The disclosed range estimate has not been adjusted for the applicable geographic differential associated with the location at which the work is being performed.
