Location: Medical Center Office Building (901 7th Ave) Department: Homeless Initiative Shift: First Shift (United States of America) Standard Weekly Hours: 40 Summary: This position is responsible for transporting patients and families from various FW shelters to their appointments at CCHCS Medical Providers. This position may also transport to Pharmacies and other locations as directed by the Case Manager for the shelters. Education & Experience
