REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

Reports to Director of Clinical Pharmacy or Local Hospital Director

 

 POSITION SUMMARY

Performs clinical pharmacy duties under the direction of the Director. The Clinical and Operational Clinical Pharmacist IV support the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, appropriate cost-effective medication therapies.

Clinical Pharmacy Practice

  1. Performs all duties of Clinical Pharmacist I, II, and III.
  2. Communicates effectively orally and in writing within department as well as to all external customers.
  3. Establishes productive, collaborative relationships with staff and community members.
  4. Exhibits professional behavior in all interactions.

 

Supervision, Oversight, and Training

  1. Contributes to multi-level education development and deployment.
  2. Precepts pharmacy learners as applicable.
  3. Incorporates training and education of frontline staff into daily workflow.
  4. Works collaboratively with all pharmacy staff to create an environment which supports optimal patient care.

 

Clinical System Pharmacist IV Additional Responsibilities

  1. Provides antimicrobial therapy oversight.
  2. Provides anticoagulant therapy oversight.
  3. Provides critical care medication therapy oversight.
  4. Provides and/or supervises comprehensive wrap around medication therapy services including medication history collection, inpatient reviews, medication stewardship, patient education, prior authorization facilitation, referral procurement, and discharge medication reconciliation preparation via system process.
  5. Assists and supports the development, implementation, and management of clinical pharmacy services through idea sharing, process development, and feedback review.
  6. Provides ongoing clinical drug information consultation and clarification to practitioners.
  7. Assists in the completion of drug information requests escalated from clinical pharmacist I or II. Escalates requests appropriately if needed.
  8. Responsible for MEC/P&T subcommittee preparation work (formulary reviews, MUEs, research, etc.) within corresponding service line.


REQUIRED QUALIFICATIONS

Education: PharmD degree or BSPharm with post-2006 experience plus PGY1 Pharmacy Residency or equivalent experience.

Clinical Skills: Patient counseling, patient profile review, pharmacokinetic evaluation, superior time and project management, preceptorship, antimicrobial basic skills, evidence-based literature review

Operational Skills: narrow service area expertise for drug information needed for preparation, administration and delivery of medications, pharmacokinetic evaluation necessary for narrow service area, superior time and project management, evidence-based literature review

Years of Experience: 3 years full time inpatient pharmacy practice License: Must be a registered pharmacist, licensed in good standing in Michigan and Ohio. Single facility-based pharmacists must retain state licensure per site location.

Certification: BLS, ACLS and/or PALS, Board Certification within 2 years of hire

 

PREFERRED QUALIFICATIONS

Education: PharmD degree or BSPharm with experience since 2006 plus PGY2 Pharmacy Residency or three years of full-time experience as a pharmacist in an acute care facility

Clinical Skills: Scientific writing, antimicrobial stewardship, critical care medication management, emergency response supportive care, education content development

Operational Skills: precepting experience, antimicrobial basic skills applicable to narrow service area, education content development

Years of Experience: 4 years full time inpatient pharmacy practice

License: no additional preferred

Certification: Board Certification

 

WORKING CONDITIONS

Personal Protective Equipment: provided based on patient risk type and material risk type.

Physical Demands: Must be able to lift light materials. Non-material handling of continuous sitting and reaching; frequent standing; occasional squatting, kneeling, walking, and stairs. Must be able to move about and between workstations.
