Location: Calmont Operations Building Department: Star Kids CM Shift: First Shift (United States of America) Standard Weekly Hours: 40 Summary: Additional Information: Performs case management/service coordination activities that enable Cook Childrens Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy, assessment, planning, communication, education, resource management, care coordination, collaboration, and service facilitation. This is an office-based position with required field work. Driving and in home visits are required up to 75% of the time. Some weekends may be required to meet member needs. Education and Experience:
Required skills: relationship-building, effective written and verbal communication skills that allow clear and concise presentation of conclusions and recommendations to a variety of audiences, critical thinking and problem-solving, ability to plan and organize effectively, proficient in typing and in use of computer applications (Excel and Outlook).
