PRIMARY PURPOSEResponsible for learning special skills and competencies in the ambulatory and acute care health system pharmacy practice while participating in a twelve-month program of structured post-graduate education and service experiences in order to gain first-hand knowledge of pharmacy practices.MINIMUM SPECIFICATIONSEducation- Must have a bachelor's degree or PharmD degree from an accredited school of Pharmacy.Experience- None.Certification/Registration/Licensure- Must be a registered pharmacist in the State of Texas or become registered within the initial three-month appraisal period.Skills or Special Abilities- Must have good oral and written communication skills.- Must have knowledge of all drugs, procedures, and equipment used in the operations of the pharmacy.- Must have good organization skills.- Must be detail oriented.- Must be able to demonstrate leadership skills.- Must be able to demonstrate cross-cultural sensitivity.- Must be able to demonstrate knowledge and skill in meeting age-related and handicapped patient's needs.- Prefer ability to speak a second language.Responsibilities1. Attends training and develops competency in health system pharmacy practice areas which include acute patient care, ambulatory patient care, drug information and drug use policy, and practice management to improve knowledge of pharmacy services and practices.2. Develops pharmacy practice skills to ensure medication, patient and employee safety at all times. Provides and assists with safety-related process improvement initiatives.3. Develops and maintains pharmacist role as an integral ember of the healthcare team by providing pharmaceutical care in an evolving, diverse, integrated health care system.4. Identifies, organizes, implements, and individually analyzes a major residency project which contributes to the enhancement of pharmacy services. Attends and presents residency project at state, local, and national professional meetings to update skills and interact with other professionals within the practice of pharmacy.5. Develops role as an efficient and effective pharmacist while staffing acute care and ambulatory practice areas to include: a. Evaluates order and prescriptions written by the medical staff and other authorized prescribers. Performs prospective comprehensive review of patient's prescription orders and profiles in order to identify potential drug related problems. b. Actively participates in resolving identified potential drug related and other problems that may improve the outcome of patient care. c. Develops and utilizes practice management skills in daily role and new program development. d. Assumes managerial responsibilities by performing weekend on-call duties. Analyze and resolve issues that may arise while on-call by consulting with coordinators and managers.6. Represents the Department of Pharmacy in a professional manner with peers, internal and external customers and other contacts. Maintains a positive working relationship with pharmacy management and personnel, other hospital employees and drug vendors.7. Assists with the department's CQI process by investigating patient complaints and developing criteria to improve systems. Identifies ways to improve work processes and improve internal and external customer satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall galls and missions of PPHS to improve department efficiency and productivity.8. Assumes responsibility for own professional growth and development by attending seminars and workshops, reading professional journals, maintaining professional affiliations, and maintaining current registration as a pharmacist in the State of Texas in order to improve professional knowledge and maintain certification required by law.9. Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the Pharmacy area. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Seeks advice and guidance as necessary to ensure proper understanding.10. Attends Pharmacy and Therapeutic Committee meetings. Assists with formulary-related projects (i.e. monographs) and provides recommendations for formulary enhancements.
