Description

Provides case management consultation serving the Dallas and Eligible Metropolitan Area (EMA) - and Dallas/Sherman-Denison HIV Service Delivery Area (HSDA). Serves as the HIV medical and non-medical case management subject-matter expert. Oversees the local Ryan White case management system of care; provides support and oversight for case management providers; ensures quality case management services are provided to Ryan White clients in the region.

Management Scope: Supervises exempt and non-exempt staff.

Responsibilities

1. Provides clinical case management consultation and technical assistance by phone or in-person to sub-recipient provider agencies for Ryan White and Ending the HIV Epidemic-funded programs and medical providers to ensure the quality of care, case management, and sub-recipient compliance.

2. Develops, in collaboration with the Administrative Agency (AA) staff, internal and external Clinical Quality Management Committee (CQM), and facilitates the ongoing improvement of the case management system to include analyzing policies and procedures, standards, complex medical assessments, acuity tools, and data management in a coordinated effort to ensure the process considers the needs of all eligible Dallas EMA/HSDA clients.

3. Develops training materials in partnership with licensed case management supervisors for providing technical assistance and training to sub-recipients. Provides guidance on obtaining HIV medical history, treatment history, medication adherence, and clinical status such as CD4 count, viral load, and other relevant tests in vulnerable populations.

4. Coordinates with partner agencies to create and manage a uniform intake and referral system that includes crisis assessment and referrals based on the needs of people with HIV. Documents linkages created between case managers and/or disease intervention services to monitor ongoing system improvement.

5. Serves as the subject matter expert for complex HIV medical and non-medical case management issues for all 12 Ryan White and Ending HIV Epidemic-funded agencies. Coordinates and oversees implementation and evaluation of restructuring of the region's case management program.

6. Performs other duties as assigned.

Qualifications

Education, Experience and Training:
Education and experience equivalent to a Bachelor's degree from an accredited college or university in Nursing, or in a job-related field of study. Three (3) years of nursing experience, including one (1) year supervisory experience.

Advanced proficiency with Bio-psycho-social assessment methodologies, case management (medical and non-medical) methodologies, and the use of electronic databases for tracking case management activities and client outcomes.

Special Requirements/Knowledge, Skills & Abilities:
Must be licensed as a Registered Nurse in the State of Texas. Must possess a valid Texas Driver's License, with a good driving record. Knowledge of HIV-related medical and non-medical case management systems. Ability to work effectively with diverse populations, including individuals affected by HIV, healthcare professionals, and community leaders. Great communication and interpersonal skills, capable of effective written and verbal communication with others, with the goal of building strong collaboration and resource networks.
