Brief Description

Performs highly complex managerial work overseeing the operational planning, establishment, execution, and evaluation of the Chronic Disease Prevention Program (CDPP); oversees planning, operational, administrative, and human resources management of the CDPP; seeks and develops outside funding sources, serves as the principal point of representation and liaison with external stakeholders on operational matters, and provides day-to-day technical/professional guidance and leadership as appropriate to the area of expertise. Oversees all aspects of the assigned programs quality assurance and training within the department.Management Scope: Supervises exempt and non-exempt staff.

Detailed Description

Plans, organizes and directs the day-to-day operations of the HHS Chronic Disease Prevention Program (CDPP), including hiring, training and staff evaluation, by developing and implementing policies and procedures, establishing long- and short-range goals and objectives. Provide guidance, mentorship, and supervision to staff within the CDPP, fostering a culture of excellence, accountability, and collaboration.Develops operating budgets in conjunction with Financial Administration and provides overall direction for the programs under purview. Manages contracts, grants, and/or state funding; approves and monitors expenditures; works with Financial Administration for budget revisions; oversees, coordinates and/or assists with proposal writing to develop/seek additional funding.Contributes to the development of a comprehensive strategic plan to address chronic diseases, outlining goals, objectives, and measurable outcomes. Advocate for policies that support chronic disease prevention and management at the local, state, and national levels. Engage with community members, organizations, and key stakeholders to raise awareness, gather input, and build support for chronic disease initiatives. Assists with oversight of Community Health Needs Assessment (CHNA) implementation. Engage with community partners, stakeholders, and organizations to build relationships and coordinate efforts to improve public health outcomes. Prepare regular reports for senior management and external stakeholders, summarizing program performance, challenges, and recommendations.Oversee the collection, analysis, and interpretation of data related to chronic disease trends and outcomes; develops and presents comprehensive statistical and narrative program reports. Utilize data to inform decision-making, evaluate program effectiveness, and identify areas for improvement. Performs community liaison and outreach activities for program(s) or project(s). Hosts regular project meetings and delivers status updates to all stakeholders.Performs other duties as assigned.

Job Requirements

Education and experience equivalent to a Bachelor's degree from an accredited college or university in Public Health, Public Administration, in a job related field of study. Five (5) years of professional work-related experience, including two (2) years supervisory experience. Skilled in the use of standard software applications. Ability to effectively communicate, both verbally and in writing, and establish and maintain effective working relationships with employees, departments and the general public. Must possess a valid Texas Driver's License, with a good driving record. Standard office environment. Ability to lift up to 25lbs., unassisted. May require prolonged sitting or standing. Ability to travel to various work site locations.
